(Exempt) FT/PT/PRN: Full time Shift: Monday - Thursday 1pm - 11pm
Salary Range: $29.41 - $41.32 per hour Location: Flat Rock, NC
Position Summary: Provides direct nursing care and instructs the
LPN and CNA staff in the delivery of nursing care to patients via
phone or telehealth. Essential Duties & Responsibilities: Practice
within the scope of license, certification or registration, where
applicable, and as required by law and regulation Serve as first
responder for symptom-based calls from current patients after
normal office hours Provide guidance to patients and families as
needed or communicate need for case manager or designee follow up
promptly Participate in implementation and coordination of
patient’s hospice care Assist with emergency admissions Assess the
patient's physical and mental health including the patient's
reaction to illnesses and treatment regimens. Record and report the
results of the nursing assessment. Plan, initiate, deliver, and
evaluate appropriate nursing acts. Teach, assign, delegate to or
supervise other personnel in implementing the treatment regimen.
Collaborate with other health care providers in determining the
appropriate patient and family-centered plan. Implement the
treatment and pharmaceutical regimen prescribed by any person
authorized by State law to prescribe the regimen. Provide teaching
and counseling about the patient's health. Report and record the
plan for care, nursing care given, and the patient's response to
that care. Supervising, teaching, and evaluating those who perform
or are preparing to perform nursing functions and administering
nursing programs and nursing services. Providing for the
maintenance of safe and effective nursing care, whether rendered
directly or indirectly. Knowledge, Skills & Abilities: Must be
willing and able to care for patients with infectious disease
processes unless approved accommodations Knowledge of Federal,
State and Local regulations and Medicare Conditions of
Participation Knowledge of HIPAA privacy rules and regulations
Knowledge of electronic medical records systems Demonstrate basic
data entry skills Demonstrate excellent organization skills

Participation Minimum Qualifications: Experience: Two years of
clinical experience preferred Licensures & Certifications: (if
applicable) Valid NC RN license Valid Driver’s License and proof of
automobile insurance Summary of Physical Requirements: Walking,
standing, sitting, bending, reaching, twisting,
crouching/squatting, kneeling Flexion/Extension of arms/wrist Hand
dexterity to include gripping, pushing/pulling and writing
Lifting/carrying of 50 lbs. often Intent & Function: All
