Job Description Job Description Qualifications & Requirements Education Graduate from an accredited school of Physical Therapy. Experience Must have current Physical Therapy license in the State of employment. Minimum 1 full year Adult Physical Therapy Home Health Experience 8-hour days- 40 hours weekly Must be willing to travel up to 75 miles from home clinic. Must be proficient in OASIS and used it within the last 6 months, or a retraining course must be completed during compliance. Key Responsibilities Performs patient evaluations and re-evaluations and documents in accordance with licensure, scope of practice and department standards Advocates for and is active in developing disciplinary and interdisciplinary team members. Ensures they are able to go anywhere in the house as needed. Verifies and reviews all provider orders and applies all pertinent medical information including lab values, vital signs, etc. routinely. Demonstrates knowledge and understanding of the profound and prolonged impact of chronic illness. Considers the individuals' age, physical, emotional, social, economic, cultural and vocational status. Understands the challenges an acute care episode has on a patient and family member. Demonstrates clinical competence in the administration of formal standardized tests, scales, and tools
