Job Description:

PACE (Program of All-Inclusive Care for the Elderly) program. In
this role, you will deliver compassionate, comprehensive care to
our senior patients, helping them enjoy a better quality of life
while managing their medical conditions in an outpatient setting.
This position requires a blend of nursing skills, including patient
assessment, care planning, and multi-disciplinary collaboration,
all geared towards enhancing the functional and health outcomes of
our elderly patients. Conduct initial and ongoing patient
assessments to determine nursing care needs. Develop and implement
individualized care plans, ensuring the health and comfort of
patients. Collaborate with a multidisciplinary healthcare team to
provide complete patient care. Educate patients and families on
health maintenance and disease prevention. Facilitate referrals to
other healthcare professionals as needed. Monitor patient progress
and adjust care plans as necessary. Strong clinical skills in a
healthcare setting Excellent communication and interpersonal skills
Ability to work effectively within a multi-disciplinary team
Experience with electronic medical record (EMR) systems

Registered Nurse (RN) license in the state of practice CPR/BLS
