licensed, BC/BE Primary Care Physician (IM or FM) for this growing
value-based medicine practice.This fast-growing, senior focused,
team approach to primary care provides a positive and well-balanced

for senior patients as we provide multi-disciplinary services and
coordinated care supported by analytics and community
