Conduct initial and ongoing assessments, monitor symptoms, and adjust the plan of care as needed . Case manage a dedicated caseload, acting as primary RN and main point of contact for patients and families. Coordinate with providers, social work, chaplains, and other team members to ensure holistic hospice care.
